Alveolar Ridge Preservation: OCP/Col more effective than Bio-Oss on bone

Optimizing alveolar healing: the challenge of the OCP/Col complex

Ridge preservation (RP) is a routine procedure aimed at counteracting post-extraction bone resorption, a prerequisite for implant success. While conventional bone substitutes are extensively documented, the octacalcium phosphate-collagen (OCP/Col) complex is generating increasing interest for its osteoconductive potential. However, the fine cellular and vascular mechanisms governing the early phase of bone regeneration under the influence of this material remained, until now, incompletely elucidated.

This murine experimental study aims to precisely evaluate the kinetics of bone formation as well as the local biological response induced by OCP/Col. Researchers focused on the distribution of CD31/endomucin double-positive vessels — indicators of specialized angiogenesis (type H vessels) — and on the expression of the transcription factor Osterix in extraction sockets.

The study is based on the hypothesis that OCP/Col outperforms reference materials, such as β-tricalcium phosphate (β-TCP) and Bio-Oss, by creating a superior micro-environment for regeneration. The authors test the ability of this complex to synchronously stimulate the recruitment of osteoblastic progenitors and the development of a highly functional vascular network during the first 14 days of healing.

Methodology of the experimental study

This in vivo study was conducted on a C57BL/6J mouse model to evaluate the impact of different bone substitutes on early alveolar healing. The protocol began with the extraction of the mandibular first molars, followed immediately by a ridge preservation (RP) procedure.

The experiment compared four distinct groups:

  • OCP/Col Group: octacalcium phosphate and collagen complex;
  • β-TCP Group: beta-tricalcium phosphate;
  • Bio-Oss Group: bovine-derived substitute;
  • Control group: extraction sockets without filling material.

The chronological follow-up extended over 14 days, with sample collections scheduled on days 1, 4, 7, and 14 post-extraction. Tissue analysis was based on a combined approach: micro-computed tomography (micro-CT) for bone morphometry, conventional histological examination, and fluorescence immunohistochemistry. The latter specifically targeted the distribution of CD31/endomucin double-positive vessels (characteristic of type H vessels) as well as the recruitment of Osterix-positive osteoprogenitor cells.

Morphometric performance and ridge preservation

Micro-computed tomography (micro-CT) analysis at day 14 post-extraction reveals a clear superiority of the OCP/Col complex (octacalcium phosphate and collagen) in socket management. Compared to the control group (extraction alone), the ridge preservation (RP) technique significantly reduced alveolar bone resorption.

Parameter (Day 14) OCP/Col Group β-TCP & Bio-Oss groups
Bone Volume / Tissue Volume (BV/TV) Significantly superior Lower than the OCP/Col
Trabecular thickness (Tb.Th) Significantly superior Lower than the OCP/Col
Biomaterial residues Notable absence / Bone replacement Persistence of residual granules

Vascular dynamics and cellular recruitment

The originality of these results lies in the observation of CD31/endomucin double-positive vascular signals (characteristic of type H vessels, known for their osteogenic role). These signals were detected throughout the healing process (Days 1, 4, 7, and 14).

  • Vascular density: The OCP/Col group showed a significantly higher abundance of CD31/endomucin vessels than the other experimental groups.
  • Angiogenesis-osteogenesis coupling: Osterix-positive cells (a key marker of osteoblastic differentiation) were frequently observed in the immediate vicinity of these vascular structures and the newly formed bone.
  • Early kinetics: From the 4th day, the environment created by the OCP/Col seems to favor a micro-environment conducive to the recruitment of bone progenitors via this specific vascularization.

In summary, histological and immunohistochemical data confirm that OCP/Col does not merely serve as a space maintainer, but actively stimulates early regeneration through a synergy between specialized angiogenesis and osteoblastic activity.

Superior angiogenic and osteogenic synergy

The results of this study mark a notable difference in alveolar healing kinetics. Where β-TCP and Bio-Oss leave persistent residual granules at 14 days, OCP/Col promotes extensive bone formation. Clinically, this suggests that OCP/Col does not merely maintain volume, but actively optimizes the microenvironment. The increased presence of CD31/endomucin double-positive vessels (type H vessels) coupled with high expression of Osterix-positive cells indicates efficient neurovascular coupling, essential for rapid and high-quality regeneration.

Limits and perspectives

Although the morphometric and histological data are robust, the study is limited by its murine model and its short observation period (14 days). While the acceleration of the early response is demonstrated, the long-term stability of bone volume in humans remains to be confirmed. However, compared to the traditional substitutes tested here, OCP/Col appears to offer a resorption/substitution more synchronous with the host's physiology.

Summary of results

This murine study demonstrates that at 14 days, the OCP/Col complex outperforms β-TCP and Bio-Oss with significantly higher bone volume and trabecular thickness. This performance is explained by increased stimulation of type H vessels (CD31/Emcn+) and Osterix+ osteoprogenitor cells, ensuring dense regeneration with near-total resorption of the material, unlike the persistent granules observed in the other groups.

In concrete terms, for the practitioner:

  • Prioritize biological remodeling: OCP/Col promotes near-complete bone substitution, avoiding the persistence of biomaterial granules often encountered with Bio-Oss or β-TCP during implant drilling.
  • Secure the early phase: The highly vascularized microenvironment induced by this complex from the 4th day optimizes site viability, a major asset for the stability of soft and hard tissues in the aesthetic zone.
  • Anticipate better density: The quality of the newly formed bone (thicker trabeculation) suggests an improvement in primary stability during implant placement after the alveolar healing phase.

Technical lexicon of the study

OCP/Col (Octacalcium phosphate collagen complex): Biomaterial complex composed of octacalcium phosphate and collagen. In this study, it is evaluated for its osteoconductive potential and its ability to promote early bone regeneration in extraction sockets.

CD31/endomucin double-positive vessels: Specific vascular signals associated with type H vessels, known to couple angiogenesis and osteogenesis. The study shows a significantly higher abundance of these vessels in the OCP/Col group during the healing phase.

Osterix: Essential transcription factor for osteoblastic differentiation. Its presence, observed near vascular structures and newly formed bone, serves as a marker to identify cells engaged in active bone formation.

BV/TV (Bone Volume/Tissue Volume): Morphometric micro-tomography parameter calculating the ratio between bone volume and total tissue volume. At 14 days, this ratio was significantly higher with OCP/Col than with β-TCP or Bio-Oss.

β-TCP (β-tricalcium phosphate): Synthetic bone substitute used as a comparison group. Unlike OCP/Col, residual β-TCP granules were still visible at 14 days in the extraction sites.

Trabecular thickness: Measurement of the thickness of bone trabeculae within the socket. This study reports a significantly higher thickness in the OCP/Col group, demonstrating an increased quality of regeneration at 14 days.
