PEEK and All-on-Four: The critical impact of cantilever and implant diameter
Polyetheretherketone (PEEK) is emerging as a serious alternative to metal frameworks for full-arch implant-supported prostheses, thanks to its reduced modulus of elasticity. However, this increased flexibility requires a precise understanding of biomechanical stress distribution.
This analysis focuses on the response of the cortical bone and the PEEK infrastructure to two key variables: the presence of a distal extension (cantilever) and the diameter of the anterior implants in a mandibular All-on-Four configuration.
Study methodology
The study is based on a three-dimensional finite element analysis (3D FEA). Four configurations were modeled, varying the diameter of the anterior implants (3.5 mm vs 4.5 mm) and the presence or absence of a cantilever. A static load of 300 N was applied (vertical and 30° oblique) on the premolar and molar positions to measure von Mises stresses and cortical bone deformation.
Major impact of the cantilever on the infrastructure
The presence of a distal extension is the primary factor in increasing stress. Under a premolar vertical load, the maximum stress within the PEEK framework increases by 59%, rising from 139.60 MPa to 222.24 MPa. This mechanical sensitivity highlights the importance of prosthesis geometry when using low elastic modulus materials.
Cortical bone response and implant diameter
Stress at the cortical bone crest increases by approximately 49% (18.36 MPa vs. 12.31 MPa) with the addition of a cantilever. The use of reduced-diameter anterior implants (3.5 mm) exacerbates the situation: cortical strain increases by 28% to 31% in models without a cantilever, and peaks with a 105% increase (0.0080 vs. 0.0039) in the configuration combining narrow implants and distal extension under molar loading.
