PEEK Implants: Laser Treatment to Boost Cellular Adhesion

Optimizing PEEK integration: beyond biological inertia

While polyetheretherketone (PEEK) is establishing itself as a serious alternative to titanium due to its elastic modulus close to cortical bone and its radiolucency, its chemical inertness remains a major clinical hurdle. In practice, this bio-inertness often results in insufficient integration, favoring the formation of fibrous tissue rather than true bone apposition. Current surface modification methods, such as coating deposits, present risks of chemical release or long-term delamination.

This study explores an innovative alternative: femtosecond laser surface texturing. The objective is to create controlled micro- and nanometric architectures to transform the topography of PEEK without altering its intrinsic mechanical properties or introducing toxic chemical agents. The authors test the hypothesis that these hierarchical structures can modulate cell adhesion and proliferation.

For the practitioner, the challenge is to validate whether these four laser-generated optimized geometries promote interaction with dental pulp stem cells (DPSCs), reference models for evaluating the osteogenic potential of a biomaterial prior to its clinical application in implantology or craniofacial surgery.

Femtosecond laser surface engineering

This in vitro study evaluates the impact of micro and nanostructuring of polyetheretherketone (PEEK) on cellular behavior. Researchers used medical-grade PEEK discs (Ensinger GmbH) with a diameter of 12 mm and a thickness of 5 mm, previously polished and ultrasonically cleaned in 70% ethanol and then in deionized water (10 minutes per step).

  • Laser protocol: Ti:sapphire femtosecond laser treatment (Solstice ACE) at a wavelength of 800 nm, with a pulse duration of 70 fs and a repetition frequency of 1 kHz.
  • Surface architecture: The beam was directed via a 160 mm f-theta lens (50 µm spot) to create four types of architectures optimized by raster scanning, without the addition of chemical reagents.
  • Biological model: Dental pulp stem cells (DPSC), selected for their high proliferation capacity and osteogenic potential.
  • Characterization analyses: Roughness and topography measurements by 3D profilometry, wettability evaluation by water contact angle (WCA) measurement, and scanning electron microscopy (SEM) imaging.
  • Cellular evaluation: Analysis of adhesion and morphology by SEM and confocal microscopy, complemented by metabolic activity tests to quantify proliferation.

Laser-driven surface architectures

The study validated four optimized surface architectures, generated by a high-precision Ti:sapphire laser system. The selected technical parameters include a wavelength of 800 nm, an ultra-short pulse duration of 70 fs, and a repetition frequency of 1 kHz. Thanks to a focal spot of 50 µm and a 160 mm f-theta lens, hierarchical micro- and nanometric textures were created. This non-linear absorption process allows for the modification of PEEK by limiting thermal diffusion, thus avoiding polymer degradation.

Impact on topography and wettability

3D profilometry and scanning electron microscopy (SEM) analyses confirm that laser texturing allows for precise adjustment of roughness without altering the bulk properties of the material. By modulating the power (up to 6 W), researchers observed a significant modification in wettability (measured by the water contact angle), creating a surface environment more favorable to biological interactions than untreated PEEK.

Dental Pulp Stem Cells (DPSC) response

In vitro evaluation of DPSC behavior — chosen for their high proliferative capacity and osteogenic potential — highlights several key points:

  • Adhesion and morphology: Laser-induced micro/nanostructuring actively supports cellular attachment. Observations reveal favorable cell-surface interactions, essential for tissue integration.
  • Proliferation: Metabolic activity tests confirm that textured surfaces maintain the viability and growth of DPSCs.
  • Pattern specificity: Biological response differences were noted between the four tested architectures, indicating that the geometric configuration of the laser pattern directly influences the kinetics of cellular colonization.

Analysis of results and clinical challenges

PEEK is emerging as a credible alternative to titanium alloys, particularly to mitigate the risks of hypersensitivity and the stress shielding phenomenon. However, its bio-inertia remains the main obstacle to its widespread adoption. This study demonstrates that surface texturing by femtosecond laser allows this barrier to be overcome without altering the polymer's intrinsic properties. Unlike hydroxyapatite coatings, which exhibit limited long-term stability related to coating adhesion, laser treatment creates a micro and nanometric topography directly within the material's bulk.

In vitro tests conducted on dental pulp stem cells (DPSC) are conclusive: the hierarchical structuring promotes cell adhesion, morphology, and proliferation. This biological model, chosen for its high proliferative capacity and documented osteogenic potential, validates the relevance of the approach for bone regeneration. By adjusting laser parameters, it is possible to precisely modulate roughness and wettability, creating a biomimetic environment conducive to osseointegration.

The weak point of this study lies in its exclusively in vitro nature. While the behavior of DPSCs on these optimized surfaces is promising, it does not fully predict the complex immune response or the final bone-implant contact under real physiological conditions. Nevertheless, the absence of toxic chemical reagents and the reproducible nature of the process make it a "clean" and industrializable technology for the design of personalized implants.

Summary of the results of this study

This study demonstrates that femtosecond laser texturing enables the creation of precise micro and nanostructures that transform inert PEEK into a bioactive substrate. In vitro evaluation of four optimized surface architectures reveals a significant improvement in the adhesion and proliferation of dental pulp stem cells (DPSCs), with notable variations in biological response depending on the specific texture applied.

In practical terms, for the practitioner:

  • Alternative to titanium: The PEEK modified in this way becomes a serious option for patients with hypersensitivity to titanium, while offering a modulus of elasticity closer to cortical bone.
  • Enhanced biological safety: Prefer femtosecond laser-treated surfaces over chemical coatings; this physical process eliminates the risks of residual toxicity and delamination of surface layers.
  • Integration customization: Implant topography is a direct clinical lever: the choice of texture influences the kinetics of cellular colonization from the earliest phases of healing.

Technical lexicon of the study

PEEK (Polyetheretherketone): High-performance polymer used for craniofacial and orthopedic implants. Its modulus of elasticity is close to that of cortical bone, which helps limit stress shielding at the implant-bone interface.

Femtosecond laser: Laser technology using ultra-short pulses (70 fs in this study) allowing precise surface structuring without thermal diffusion. This process avoids thermal degradation of the polymer while creating micro and nanoscopic textures.

Stress shielding effect: Phenomenon of bone density reduction surrounding an implant, caused by the excessive rigidity of the latter (notably with titanium). PEEK is being studied as an alternative to minimize this effect thanks to its more physiological mechanical properties.

DPSCs (Dental Pulp Stem Cells): Dental pulp stem cells. These are a population of mesenchymal stem cells with a high proliferation capacity and osteogenic differentiation potential, used here as an in vitro biological model.

Wettability: Physical property of the surface measured by the water contact angle (WCA). Femtosecond laser modification allows for the adjustment of this wettability to influence protein adsorption and initial cellular adhesion.

Hierarchical structuring: Surface architecture combining micro-scale and nano-scale patterns. These structures mimic the natural bone environment to promote cellular anchoring and osseointegration of the inert biomaterial.
